Baptist Memorial Hospitals - E-Patient
As a Baptist e-patient, you can preregister online from the comfort of your home. Besides being more convenient, registering online can save you time. At Baptist, we want to make preparing for your hospital visit or outpatient procedure, such as a mammography, radiology or lab test, as simple as possible. Please preregister online at least 72 hours before your visit to the hospital or outpatient unit to allow adequate time for processing your information.



What you'll need to preregister:
  • Your personal contact information
  • Emergency contact information
  • Insurance information (carrier, group name and number, policy number, address and telephone number)
  • Physician name
  • Date of visit/procedure/test

The preregistration form is designed to work with browser versions Internet Explorer 5 or greater, and on operating systems Windows 98, ME, 2000 or XP.

After you preregister, please bring the following items with you to check in at registration:
  • Insurance card
  • Photo ID
